Insurance and Billing
At the time of your visit, you will need to provide the front desk with your current insurance information, including your insurance card(s) to be copied. We will perform courtesy billing for you, provided you have given us all pertinent billing information. You are responsible for full payment of any co-payments, co-insurance and deductibles required by your insurance company.
We also offer the option to pay bills online. With our online bill pay option, you can securely make a payment for your service online with no hassles.
Pay Bill OnlineYour insurance coverage is an agreement between you and your insurance company. Financial responsibility rests with the patient for deductibles, co-insurance and non-covered services.
In addition to your bill from the center, you may receive a bill from other entities. Click here for a list of possible service providers that may charge you.
If your insurance requires authorization for an office visit, please contact your referring physician for that authorization prior to calling for an appointment.
